29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level were not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. A. Employees who operated a grinder to recycle plastic were exposed to a fall hazard. An elevated platform approximately 4.5 feet above the concrete floor lacked standard railing along some areas of the platform. B. Employees who operated a grinder to recycle plastic were exposed to a fall hazard. An elevated platform approximately 4.5 feet above the concrete floor where the operator stood when feeding the grinder was equipped with a top rail, but an intermediate rail was not installed to meet the requirements of paragraph (e)(3) of this section.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Employees who used a fixed stair to access the Inventory Room were exposed to a tripping hazard. A compressed air hose was strewn on the floor at the bottom of the stairway.
