29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) At the job site, on September 12, 2013, the employer did not ensure the top rung of a 300 pound rated, six-foot, fiberglass constructed stepladder was not used by an employee who was performing framing work directly over a concrete slab, exposing the employee to the hazards associated with death, broken bone, concussions, lacerations, and bruises.
