29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a) Location: 339 Atlantic Avenue, Marblehead MA, 01945-2762 On or about August 16, 2013, a competent person did not perform a frequent and regular inspections of the job site to identify safety and health hazards such as a damaged/defective aluminum extension ladder being use by workers to carry materials to and access the roof, defective electrical cords, workers without fall protection, fall anchors removed from structure, and electrical cords not GFCI protected exposed to pooling and spraying water..

(a) Location: 339 Atlantic Avenue, Marblehead MA, 01945-2762 On or about August 16, 2013, a portable aluminum ladder that was damaged/defective was not withdrawn from service and tagged to identify it as defective.
