29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a) On west side of house: On or about May 1, 2013 and at times prior to, the employer exposed his employees to fall hazards in that he allowed his employees to work on a 8:12 pitch roof without fall protection exposing his employee to falls of approximately 8 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about May 1, 2013 and at times prior; the employer failed to ensure a portable extension ladder used on site by employees was extended at least three feet above the landing area exposing them to fall hazards of 8 feet to the ground below.
