29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: 4404 Sentry Drive, Tucker, GA (Worksite) - The employer is not entering recordable injuries and illnesses onto the OSHA 300 logs within seven calendar days but is waiting to record them at the end of the year for calendar year 2013; the following were not recorded onto the OSHA-300 logs, on or about September 10, 2013. a. On or about May 7, 2013, Machine Operator - an employee cut finger on the Multi-Vac Machine. This injury resulted in the employee receiving prescription medication from the physician. b. On or about June 20, 2013, Machine Operator - an employee cut finger on the Tray Line Blade. This injury resulted in several days of restriction and several days away from work until July 22, 2013, per physician orders. c. On or about June 24, 2013, an employee struck and/or injured left hand while pushing the buggy. This injury resulted in several days away from work until June 26, 2013, and receiving prescription medication from the physician.
