29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) 901 East 18th Street, Ductbank Detail 2/E-003 - On or about September 11, 2013, an employee who was working in the bottom of a 7.5-foot deep trench was not provided with adequate training about hazards associated with excavation work and the necessary precautions to prevent unsafe conditions such as but not limited to cave-in hazards, resulting in an engulfment and crushed-by hazards. ABATEMENT CERTIFICATION IS REQUIRED

ABATEMENT CERTIFICATION IS NOT REQUIRED
