29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At this skilled nursing facility, employees performing housekeeping and maintenance work, were not provided with eye and face protection when working with potentially corrosive chemicals. Employees use liquid chemicals such as, but not limited to:  A. Chlorine bleach.  B. Tri-Star Neutralizer.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure:Note:  Does not apply to employees who have previously received the complete hepatitis B vaccination series, for whom antibody testing has indicated immunity, or for whom the vaccine is contraindicated for medical reasons.   At this establishment, where employees are exposed to bloodborne pathogens, the Hepatitis B series vaccination was not made available within 10 days of the initial assigments involving occupational exposure.
