29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Jobsite - on or about September 11, 2013 - an employee was performing sandblasting of the 12 inch diameter natural gas pipe, working in an excavated trench that measured approximately 10 feet wide by 15 feet long by 5 feet, 7 inches deep. No protective systems were in place to prevent a cave-in hazard of this trench.
