29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) First General Services of Western PA / 712 Spearman Ave, Farrell, PA: On or about September 13, 2013, personnel were exposed to falls of approximately 68-1/2 inches from the hill/retaining wall area and the pallet utilized to walkout over and to the dumpster. Personnel could fall between the hill and the dumpster and/or into the dumpster.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) First General Services of Western PA / 712 Spearman Ave, Farrell, PA: On or about September 13, 2013 employees had not received hazard communication training. Employees work with a variety of materials that range from glass cleaner, stainless steel cleaners, adhesives, glues, strippers, paints and paint thinners.
