29 CFR 1910.178(m)(6): The employer did not ensure that safe distance was maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car: (a) On or about September 16, 2013 the inspection revealed that employees were exposed to serious injury from the use of a loading dock that did not have a vehicle curb, bull rail or other effective barrier to prevent vehicles and forklifts from going over the edge. The loading dock was located outside of the southeast corner of the lower maintenance shop at the John Day Dam in Rufus, Oregon.

29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: (a) On or about September 10, 2013 the inspection revealed that employees were exposed to serious injury from a pipe chase pit with the cover removed that was not adequately protected. The pipe chase was located in the tanner valve #4 storage room at the John Day Dam. (b) On or about September 16, 2013 the inspection revealed that employees were exposed to serious injury from pit openings that were not protected by a floor cover or guardrails. The pits was located in the level 131 fish turbine room at the John Day Dam. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 730483, citation number 1, item number 1 and was affirmed as a final order on April 24, 2013, with respect to a workplace located at The Dalles dam in the The Dalles, Oregon.

The hazard was located on the powerhouse roof of the John Day dam located near Rufus, Oregon. (b) On or about September 11, 2013 the inspection revealed that employees were exposed to a serious injury from a fall of about 5'7" at turbine generator #7. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 96892, citation number 1, item number 4 and was affirmed as a final order on September 15, 2012, with respect to a workplace located at McNary Dam in Umatilla, Oregon. The horizontal shaft was part of pump #1 at elevation 154 located at the John Day Dam. (b) On or about September 16, 2013 the inspection revealed that employees were potentially exposed to serious injury from a horizontal shaft that was not fully guarded on the Cincinnati Bickford Radial drill located in the lower machine shop at the John Day Dam. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 682538, citation number 1, item number 2 and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 8010 NW Saint Helens Road in Portland, Oregon.
