Section 5(a)(1) of the Occupational Safety and Health Act of 1970:     The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to a fall or crushing hazards while being raised, lowered, and working in the elevated front end loader bucket to perform work such climbing out to level loads in a trailer and to secure scrap loads on trailers:       (a)  Scrap yard area: Employees were exposed to falls of approximately 13 feet and crushing hazards between the bucket and scrap trailer while being lifted and lowered in the bucket of an International 530 front end loader, as determined on September 9, 2013.       Abatement Certification and Documentation Required for this Item.

29 CFR 1910.151(b):    There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:      a. Scrap yard: Employees were exposed to injuries such as, burns, cuts, scraps and broken bones while working in the scrap yard.  An infirmary, clinic, or hospital was not in near proximity to the workplace and a person was not trained to render first aid, as determined on September 12, 2013. 29 CFR 1910.253(b)(4)(iii):    Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:       a.  Fabrication shop cylinder storage area  Employees store full and empty oxygen cylinders in the cylinder storage area.  A propane bottle was stored with the oxygen cylinders, as observed on 10/15/07.
