29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually: (a) Golden Living Center- The employer did not ensure that the annual review of the blood borne pathogen exposure control plan was documented. The program did not include a revision date. (b) Golden Living Center- The employer did not ensure that the annual review of the bloodborne pathogen exposure control plan documented the implementation of safe sharp devices by listing the Safe sharpdevices in use at the facility. (c) Golden Living Center- The employer solicited input from non-managerial employees who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls but did not document the solicitation procedure in the exposure control plan. Golden Living Center Dunwoody was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(c)(1)(iv), which was contained in OSHA inspection number 892807, citation number 1, item number 1 and was affirmed as a final order on June 24, 2013, with respect to a workplace located at 613 Roselane Street NW, Marietta, GA. 30060.

29 CFR 1910.1030(h)(5)(i): The employer did not establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps: (a) Golden Living Center Dunwoody- A 2013 Sharps Injury Log was not established and maintained for two contaminated needle-stick injuries that occurred on April 28, 2013 and August 14, 2013. The injuries were recorded correctly without personal identifiers on the employers 2013 OSHA 300 Log and their 2013 Privacy Case Log. The sharps injury log should include the type and brand of device involved in the incident; the department or work area where the exposure incident occurred; and an explanation of how the incident occurred to assist the employer in deciding if the device was part of the cause. (None of this information was contained on the Privacy Case Log) The Golden Living Dunwoody was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(h)(5)(i), which was contained in OSHA inspection number 892807, citation number 1, item number 2 and was affirmed as a final order on June 24, 2013, with respect to a workplace located at 613 Roselane Street NW, Marietta, GA. 30060.
