29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       (a) 2228 Quail Run Road, State College Pennsylvania - The employer did not provide fall protection for employees performing roofing work on the roof of a residence. The roof pitch was 6 in 12, the pitch of the dormers was 10 in 12.  Employees were exposed to a fall of 8 feet 6 inches from the eave to the ground below on or about September 16, 2013.      No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:     (a) 2228 Quail Run Road, State College Pennsylvania - Employees used a portable ladder to access the roof, the side rails did not extend three feet above the upper landing surface.  The employees were exposed to a fall of 9 feet from the roof to the ground below on or about September 16, 2013.     No abatement certification or documentation required.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  (a) 2228 Quail Run Road, State College Pennsylvania - Employees used a ladder that was not secured to prevent accidental displacement.  They ascended the ladder  to work on the roof and were exposed to a fall of 9 feet from the roof to the ground below on or about September 16, 2013.  No abatement certification or documentation required.
