29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Roof, located at 1223 Poston Place, Smyrna, GA 30080: Employees using positioning belts/mountain climbing harnesses on a 6/12 pitched roof at heights of approximately 30-feet, 9-inches used an inadequate fall arrest system when the attachment to the body harness was not made on the wearer's back. Employees attached life-lines to carabiners located in the front webbing of the mountain climbing harness which is located at the wearer's waist. A positioning device can not be used as a personal fall arrest system.
