29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: (a) On or about September 5, 2013, and at certain times before, an employee operating the "envelope stuffing machine" was exposed to potential entanglement and/or possible amputation type injuries due to the employer not having developed and implemented an effective energy control procedure.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, or flying chips and sparks: (a) On or about September 5th, 2013, an employee operating the "envelope stuffing machine" was exposed to several hazards from unguarded moving parts due to a lack of a protective guard surrounding the component's located beneath the working table of the machine. The hazardous components (which were integral to the machine) included unguarded chains and sprockets, gears, rotating parts, as well as unguarded belts and pulleys.
