29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) On or about September 18, 2013, at address of 1068 Hotel Drive in Defiance, OH, the employer failed to ensure that employees were protected from falls while working from a tubular welded frame scaffold. Employees were working from the scaffold platform without an acceptable form of fall protection, exposing them to fall of approximately 18 feet to the ground below.
