29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards):  (a) At the site, located at 14925 Memorial Dive in Houston, Texas, on or about 09/18/2013, frequent and regular inspections of the job site were not conducted by a competent person.  Employees were exposed to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a)  At the site, on or about 09/18/2013, employees cutting decking pieces were exposed to fall hazards of approximately 23.5' while working without fall protection.
