29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods. On or about September 17, 2013, lock out procedures were not used for the control of hazardous energy while entering the grain storage bins, exposing the employee to the hazard of being caught-in the rotating parts of the floor auger.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating. On or about September 17, 2013, employees were operating equipment associated with the grain bins that had broken electrical equipment, in several instances including but not limited to: a) 2 breaks in the conduit to the auger switchbox. b) 2 breaks in the conduit leading to the auger motor. Abatement note: Frequent inspections should include identifying and correcting defects in electrical wiring, switches, boxes and equipment.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets of other forms of approved enclosures, or other means listed under this provision: On or about September 17, 2013, employees were accessing the breaker panel that provides 120 volt power to the grain bins without a cover in place to protect employees from live parts, exposing employees to an electrical shock hazard.
