29 CFR 1910.181(c)(1): Permanently installed derrick(s) with fixed lengths of boom, guy, and mast were not provided with substantial, durable, and clearly legible rating chart(s) securely affixed where visible to personnel responsible for the safe operation:  Well site:  On or about September 17, 2013, and times prior to, an operator used a gin pole(derrick) truck to lift and load a well control assembly.  There was no load rating information either posted or available for operator, exposing the employee to the hazard of being struck by the load or a malfunctioning gin pole.

29 CFR 1910.181(d)(3)(i): Complete inspections of the derrick were not performed at intervals as generally defined in paragraph (d)(1)(ii)(b) of this section depending upon its activity, severity of service, and environment, or as specifically indicated:  Well site:  On or about September 17, 2013, and times prior to. an operator used a gin pole(derrick) truck to lift and load a well control assembly.  There were no inspection conducted, exposing the employee to the hazard of being struck by the load or a malfunctioning gin pole.

29 CFR 1910.181(f)(3)(iii): Repairs or replacements were not provided promptly as needed for safe operation of derricks:   Well site:  On or about September 17, 2013, and times prior to,an operator used a gin pole(derrick) truck to lift and load a well control assembly.  The gin pole running ropes were severely damage, exposing the employee to the hazard of being struck by the load or a malfunctioning gin pole.

29 CFR 1910.181(g)(1): The employer did not ensure that thorough inspection of all ropes in use was made at least once a month nor that a certification record which included the date of inspection, the signature of the person who performed the inspection, and an identifier for the ropes which were inspected was prepared and kept on file where readily available:   Well site:  On or about September 17, 2013, and times prior to, an operator used a gin pole(derrick) truck to lift and load a well control assembly.  Wire rope inspections were not conducted and wire ropes were damaged exposing the employee to the hazard of being struck by the load or a malfunctioning gin pole.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:  Well site:  On or about September 17, 2013, and times prior to, an employee operated a gin pole truck during the moving and loading of a well control assembly using the lifting line tail chain.  The chain was missing its information tag exposing the employee to the hazard of a falling load.

29 CFR 1910.184(e)(3)(i): Thorough periodic inspections of alloy steel chain slings in use were not made on a regular basis:  Well site:  On or about September 17, 2013, and times prior to, an employee operated a gin pole truck during the moving and loading of a well control assembly using the lifting line tail chain.  The sling had not been inspected exposing the employee to the hazard of being struck by a falling load.
