29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    a) Summit Hill, PA: A worker removing windows from a structure that was being demolished was exposed to a fall while standing on a piece of plywood that was resting approximately 24 feet in the air on the forks of a Gradall rough terrain forklift, on or about September 19, 2013.
