29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)  Force Account Building, on or about September 18, 2013, and at times prior thereto, a flexible extension cord was used as permanent wiring from the power pole outlet to the outlet on the backside of the building.  Note:  Abatement certification is required for this item.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:  (a)  Force Account Building, on or about September 18, 2013, and at times prior thereto, a flexible extension cord used as permanent wiring from the power pole outlet to the outlet on the back side of the building, was damaged and had not been removed from service.  Note:  Abatment certification is required for this item.
