29 CFR 1926.1412(d)(1)(xii): The employer did not inspect the operator cab windows for significant cracks, breaks, or other deficiencies that would hamper the operator's view: (a). The front windshield of Crane #75 that was being used at the Woodsmere Townhome development in Ocoee, Florida, had spiderweb cracks radiating from the windshields right center up and down the right side as observed on or about September 17, 2013.

29 CFR 1926.1412(h): Equipment not in regular use. Equipment that has been idle for 3 months or more must be inspected by a qualified person in accordance with the requirements of paragraph (e) (Monthly) of this section before initial use. (a). The Crane Unit #75 that was being operated at at the Woodsmere Townhome development in Ocoee, Florida, did not have had not been inspected. No Frequent Inspection, Periodic Inspeciton or Annual Inspectioin was conducted or documented to be presented at the time of request on or about September 17, 2013.
