(b) Northfield Unit; Operating equipment was outside acceptable limits as there was excessive ice buildup on the Northfield Surge Drum (URH-2-VES-060) and associated valves (URH-2-SRV-2108, URH-2-SRV-2105 and URH-2-SRV-2104) which would interfere with closing the valves in a timely manner in an emergency situation.
