29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before further use to ensure safe operation of the equipment: (a) Northfield Unit; Operating equipment was outside of acceptable limits as there was excessive ice buildup on the Northfield Oil Pot (URH-2-VES-061) and associated valve (URH-2-SRV-2103) which would interfere with closing the valve in a timely manner in an emergency situation. (b) Northfield Unit; Operating equipment was outside acceptable limits as there was excessive ice buildup on the Northfield Surge Drum (URH-2-VES-060) and associated valves (URH-2-SRV-2108, URH-2-SRV-2105 and URH-2-SRV-2104) which would interfere with closing the valves in a timely manner in an emergency situation.
