29 CFR 1910.179(i): Crane(s) equipped with a power traveling mechanism were not provided with a gong or other effective warning signal:  On or about September 18, 2013, and at times prior to, at the facility overhead cranes were operated by a remote control device to lift heavy steel parts were not provided with the means (audible alarm or strobe light) to warn employees working in the areas when the cranes were on the move.  Employees were exposed to struck-by hazards.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:   On or about September 18, 2013, and at times prior to, in the center bay areas steel chain slings being used with overhead crane to lift heavy metal materials were observed not having identification tag affixed stating size, grade, rated capacity, and reach.  Employees were exposed to struck-by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about September 18, 2013, and at times prior thereto, employees were allowed to operate machines that were found not having proper guards provided, including but not limited to the following instances:     1) In the R & D areas, the shear was found having the guard provided with large openings that allow hands or fingers could enter the point-of-operation.      2) At the press-brake in the west side of shop area, the employer allowed employees to operate a press-brake via foot-pedal control without proper machine guards provided.    Employees were exposed to possible amputation hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:   On or about September 18, 2013, and at times prior thereto, in the west side shop press brake area employees were exposed to a sprocket wheel and chain system approximately 2.5-ft. from the floor without a guard provided to prevent employees contacting the sprocket wheel and chain system while in operation.  Employees were exposed to an amputation caused by the unguarded sprocket wheel and chain system.
