29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas: On or about September 17, 2013, at the worksite located at 8 Deauville Circle in Little Rock, Arkansas, front of the building being constructed, wooden boards with protruding nails were scattered on the ground. This condition exposed employees to punctures.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about September 17, 2013, at the worksite located at 8 Deauville Circle in Little Rock, Arkansas, the 25 ft. fiberglass extension ladder used by employees to access and descend from the upper ceiling/floor level, extended approximately 14 inches above the landing surface. This condition exposed employess to falls from elevations.
