29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   (a). Worksite located at 150 NW US Highway 50, Warrensburg, Missouri- Employees were observed walking underneath an elevated boom.  Employees were not protected by the use of hard hats.

Worksite located at 150 NW US Highway 50, Warrensburg, Missouri- Employee was observed wearing a harness, but failed to have his lanyard connected to the boom or basket of the lift.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a). Worksite located at 150 NW US Highway 50, Warrensburg, Missouri- Employees were exposed to a fall hazard in that the employees were using a step ladder that was too short to access the interior floor of the building wall.
