An employee was exposed to amputation hazards while operating a hydraulic alligator shear without a point of operation guard. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:     a) On or about September 18,2013, and at times prior, the employer did not ensure that  flexible cords and cables were connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. A cable connecting power to a hydraulic shear electrical motor showed signs of strain at the joint. The outer cable insulation was damaged exposing the inner insulation going into the motor. This condition exposed employees to contact with electrical current hazards. 29 CFR 1910.23(d)(1)(iii): Flight(s) of stairs with 4 or more risers, less than 44 inches wide and having both sides open were not equipped with one standard stair railing on each side:    a) On or about September 18,2013, and at times prior, the employer did not ensure that a flight of stairs with 4 or more risers, less than 44 inches wide and having both sides open was not equipped with one standard stair rail on each side. A flight of stairs with 19 risers and less than 44 inches wide, used to reach a second floor office space, did not have a standard stair railing on the right side descending, exposing employees to falls from the lowest 7 risers.
