29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Ashcraft Tree Services, Inc., Findlay, Ohio:  On or about September 17, 2013, at address 846 Bright Rd., Findlay, Ohio, the employer did not ensure that each employee used appropriate eye protection while performing tasks related to tree trimming operations, such as, but not limited to clearing brush and downed tree branches and operating the wood chipper, exposing employees to eye injuries.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  a) Ashcraft Tree Services, Inc., Findlay, Ohio:  On or about September 17, 2013, at address 846 Bright Rd., Findlay, Ohio, the employer did not ensure that each employee used appropriate head protection while working below areas where tree trimming operations were taking place.  Employees were exposed to potential struck-by injuries caused by falling branches and/or equipment.
