29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) U.S.A. Roofing Inc. on or about September 18, 2013, at the address of 1 Lawrence Rd.; Port Clinton, OH: The employer failed to ensure employees were protected from falls while performing roofing work on a non-residential structure. The employees working from a steep sloped roof line were exposed to falls of approximately 10 ft. to the ground below. U.S.A. Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 311609788, citation number 1, item number 1 and was affirmed as a final order on July 30, 2009, with respect to a workplace located at 286 S. Main St.; Creston, OH.
