29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Heitkamp Homes, LLC, Findlay, Ohio:  On or about September 17, 2013, at a worksite located at 1401 W. Sawmill Rd., Findlay, Ohio, the employer did not ensure that employees were protected from falls while engaged in roofing activities on a steep sloped (6:12) residential type structure.  Employees were exposed to a potential fall of approximately 18.6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Heitkamp Homes, LLC, Findlay, Ohio:  On or about September 17, 2013, at a worksite located at 1401 W. Sawmill Rd., Findlay, Ohio, the employer did not ensure that each employee had been trained in the recognition and control of fall hazards.  Employees exposed to falls of approximately 18.6 feet were not trained to use fall protection systems, which could result in permanent disability or death.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Heitkamp Homes, LLC, Findlay, Ohio:  On or about September 17, 2013, at worksite adjacent to 1401 W. Sawmill Rd., Findlay, Ohio, the employer did not ensure that the side rails of a fiberglass extension ladder were extended at least three feet above the landing.  Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
