29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Heitkamp Homes, LLC, Findlay, Ohio: On or about September 17, 2013, at a worksite located at 1401 W. Sawmill Rd., Findlay, Ohio, the employer did not ensure that each employee had been trained in the recognition and control of fall hazards. Employees exposed to falls of approximately 18.6 feet were not trained to use fall protection systems, which could result in permanent disability or death.

Sawmill Rd., Findlay, Ohio, the employer did not ensure that the side rails of a fiberglass extension ladder were extended at least three feet above the landing. Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.
