29 CFR 1910.23(a)(6): Every manhole floor opening shall be guarded by a standard manhole cover which need not be hinged in place. While the cover is not in place, the manhole opening shall be constantly attended by someone or shall be protected by removable standard railings.  a) On or about September 19, 2013 a floor hole measured approximately ninteen inches wide was not covered to prevent someone from stepping inside of the opening.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,  a) On or about September 19, 2013 mixing area loading platform lacked standard railing  or chains to prevent a fall hazard of 6 feet to concrete floor.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard.  a) On or about September 19, 2013 the stairs to the loading platform in the Set Up/Mixing Area was not equipped with a midrail.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  On or about September 19, 2013, ten fifty five gallons of water base latex paint stored in front of a electrical panel did not permit sufficient access of working space and readily safe operation and maintenance of the electrical panel box rated at 220 volts for emergency and or quick diconnect.
