29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about September 20, 2013 at the above addressed jobsite, an employee installing a wall was exposed to a fall, of approximately thirty (30) feet, created by the presence of unprotected sides.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. On or about September 20, 2013 at the above addressed jobsite, an employee installing a wall on the roof was not protected from free falling more than six feet. The employee was exposed to the hazards associated with falls.
