Nichols Excavating @ 1800 Welton Street, Denver, CO 80204:  On and before September 13th, 2013, the exposing employer did not ensure that an employee conducting new sewer pipe installation work in an excavation was protected from cave-in, in that, the excavation was approximately 6 feet deep and lacked an adequate protective system, such as appropriate sloping or shield system. This condition exposed the employees to a crushing hazard by excavation wall cave-in.
