29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    1) Panhandle Plaza, Lot 29 Panhandle Road, LoveJoy, GA - An employee was working from a mobile scaffold system installing metal frames. The casters and wheels were not locked to prevent movement of the scaffold system while in use. An employee was pushing the scaffold while the employee was working on it. Employee was exposed to a fall hazard.

Employees were exposed to a fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    1) Panhandle Plaza, Lot 29, Panhandle Road, LoveJoy, GA - Employees were using a 7 foot Werner fiberglass step-ladder to access and egress the roof. The 7-foot Werner step-ladder was leaning against the roof and employees were using the top step to access and egress the roof that was about 2 feet below the top platform of the roof. The ladder was not designed for the purpose it was being used.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:    1) Panhandle Plaza, Lot 29, Panhandle Road, LoveJoy, GA - Employees were using a 7 foot Werner fiberglass step-ladder on top of a GMC pickup truck to access and egress the roof. Employees had to use the top step of the ladder to access and egress the roof that was about 2 feet below the top platform of the roof. The ladder was not on a stable footing. Employees were exposed to about 18 feet high.
