The employees were exposed to a struck-by hazard.

The highest part of the roof was 17 feet high and the lowest part was 10 feet to the ground. Employees were exposed to a fall hazard.

There was a 2 feet gap between the top rung of the ladder and the roof landing. Employees were exposed to a fall hazard.
