29 CFR 1910.217(d)(1)(i): The employer did not use dies and operating methods designed to control or eliminate hazards to operating personnel. FORGING DEPARTMENT: The employer did not establish appropriate procedures for the employees required to set up and/or remove the dies from power presses. On/or about 09-19-13, the die, which was removed from the Trim Press, No. F/Billing Spencer #2 fell on an employee's hand who was trying to push/load the die onto a table of the Presto #7 die cart, and the employee sustained severe hand injury, finger amputation from the incident. ABATED DURING THE INSPECTION.
