One employee was observed working from one scaffold plank approximately 12 inches in width. The employee was exposed to a fall hazard of approximately 15 feet.

One employee was observed working on the roof of a three story building. Two employees were observed working from the third floor porch roof to install exterior siding. None of these employees were using fall protection. Employees were exposed to a fall hazard of 20-30 feet.
