On or about 09/20/2013 at 6960 Oak Creek Road, Cumming, GA: The employee installing a new culvert in creek bed was exposed to a fall hazard when the only means of accessing/egressing the trench was crawling through 100-feet of a 42-inch concrete pipe.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems.       On or about 09/20/2013 at 6960 Oak Creek Road, Cumming, GA: Daily inspections of the trench were not made by a competent person. An employee was exposed to a cave-in hazard when the discrepancies found included but were not limited to:      a) an inadequate protective system. A single trench box was used to protect an employee working in a 20-foot deep trench     b) the side walls of the 20-foot deep trench showed signs of cracking and dropping back into the trench     c) no other means to prevent water accumulation other than a gravel base were placed in the trench
