29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    1) On or about September 14, 2013, and at prior times, employees were exposed to hazards while working with hydrofluoric acids when cleaning the walls of the car wash.  The employer did not provide the employees with chemical protective clothing such as aprons or coveralls, and boots.

29 CFR 1910.133(a)(1): 3 29 CFR  1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  1) On or about September 14, 2013, and at prior times, employees were exposed to eye and face hazards while working with acids and corrosives at the following locations:  a) hydrofluoric acid when cleaning the walls of the car wash and when changing bulk hydrofluoric acid containers;  b) sodium metasilicate pentahydrate when adding car wash powder to the system;  c) sodium hydroxide when conducting titer tests.  The employer did not provide appropriate eye and face protection.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  1) On or about September 14, 2013, and at prior times, employees were exposed to eye and body hazards while working with acids and corrosives at the following locations:  a) hydrofluoric acid when cleaning the walls of the car wash and when changing bulk hydrofluoric acid containers;  b) sodium metasilicate pentahydrate when adding car wash powder to the system;  c) sodium hydroxide when conducting titer tests.  The employer did not provide appropriate emergency eye wash and shower station, nor otherwise provide suitable facilities for quick drenching or flushing of the eyes and body.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    1) On September 19, 2013, and at prior times, employees were exposed to hazards where guarding was insufficient on pulleys at the following locations:      a) a pulley at the hot charge pump;      b) a pulley at the oscillating pump;      c) a pulley at the power wash.      The guarding installed would not have prevented employees from getting any part of their body into the rotating pulley.  The guarding was not of approved material and was not securely attached.
