29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety: a) Jobsite, 400 Edgemont Street, Pittsburgh, PA 15211: Observed on or about April 30, 2013, employees working in close proximity to overhead power lines while engaged in tree trimming/removal operations were not trained in and familiar with electrically safety-related work practices and safety procedures. The employees were exposed to the hazard of electric shock and electrocution while engaged in tree trimming and tree removal activities at residential property.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: a) Jobsite, 400 Edgemont Street, Pittsburgh, PA 15211: Observed on or about April 30, 2013, the employer did not contact the utility company to ensure overhead electrical lines located in close proximity to trees trimming activities. Employees removing limbs from a tree were exposed to electric shock and electrocution hazards when contact was made with an overhead power line.
