29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  This violation occurred on September 17, 2013, at a job site located at 4305 Wheatland Road, Dallas, Texas 75236; where at least four (4) employees working at ground level was not protected from the impact of falling and flying objects by the employees working above.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  This violation occurred on or before September 17, 2013, at a job site located at 4305 Wheatland Road, Dallas, Texas 75236; where at least three (3) employees worked from a single pole scaffold and were not protected from falling.
