29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  At the job site, employee(s) were unable to recognize the following hazards that they were exposed to:  a/  How to identify and when to wear safety glasses.  b/  The self-closing lids were required for all containers used for the dispensing of flammable or combustible liquids.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a/  At the job site, employees were installing new shingles at the southwest corner of the roof with Bostitch Nail Guns, m/n RN46-1, s/n 120860760T and 05277272.  These employees were not provided with or wearing appropriate eye protection.

29 CFR 1926.152(e)(5): Dispensing devices and nozzles for flammable liquids were not of an approved type:  a/  At the job site, the black, plastic nozzle belonging to a red, plastic, ten-gallon container partially filled with gasoline was not equipped with a self-closing lid.  This nozzle was not equipped with any device to prevent flammable vapors from escaping into the atmosphere.  This container was located adjacent to a running air compressor positioned along the north end of the commercial building under construction.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems (PFAS):  a/  At the job site, three employees were observed installing shingles at the southwest corner of the 6:12 sloped roof.  No form of conventional fall protection such as guardrails, safety nets, or PFAS was in use.  The ground to eave height was approximately 12 feet.
