29 CFR 1926.152(e)(5): Dispensing devices and nozzles for flammable liquids were not of an approved type: a/ At the job site, the black, plastic nozzle belonging to a red, plastic, ten-gallon container partially filled with gasoline was not equipped with a self-closing lid. This nozzle was not equipped with any device to prevent flammable vapors from escaping into the atmosphere. This container was located adjacent to a running air compressor positioned along the north end of the commercial building under construction.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems (PFAS): a/ At the job site, three employees were observed installing shingles at the southwest corner of the 6:12 sloped roof. No form of conventional fall protection such as guardrails, safety nets, or PFAS was in use. The ground to eave height was approximately 12 feet.
