29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On 9/23/13, an employee was accessing the roof of the house at 16 9th St. in Clintonville, Wisconsin by an extension ladder when the ladder did not extend 3 feet above the roof eaves.

29 CFR 1926.1060(a)(1): 1926.1060(a)(1):  The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards:  a) Employees using an extension ladder at the site to access the roof were not trained on the placement of ladders and safe work practices to avoid fall hazards when climbing and stepping to and from the work surface to the ladders.
