29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  a. 260 First Street Jersey City, NJ  Employees were observed performing siding activities from a welded tubular frame scaffold that was not fully planked between the front uprights and the guardrail supports, thereby exposing employees to 36 ft fall hazards. 29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  a. 260 First Street Jersey City, NJ - South side of the building    Employees were conducting work from a supported scaffold. Some of the supported scaffold posts were not resting on base plates.   b. 260 First Street Jersey City, NJ  South side of the building   Employees were conducting work from a supported scaffold. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   260 First Street Jersey City, NJ  Safe access and egress to the welded tubular frame scaffold was not provided to the work platform.  Employees were observed egressing a welded tubular frame scaffold work platform using the cross braces.  Violation observed on or about 07/31/2013
