29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) On the west side of the residence, employees working from a pump jack scaffold installing siding on a two-story residence were not provided with fall protection, the employees were exposed to a fall of more than 17 feet to the next lower level.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary: (a) On the west side of the residence, the employer allowed employees to work from a pump jack scaffold that did not have the bottom secured by rigid triangular bracing or equivalent means to prevent displacement, exposing the employees to a fall of more than 17 feet to the next lower level.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) On the west side of the residence, employees installing siding from a pump jack scaffold were not trained to recognize the hazards associated with the use of a pump jack scaffold and the methods to control and minimize the hazards. The employees were exposed to a fall hazard of more than 17 feet.
