29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  At the seventh floor mat room, used to train employees in defensive tactics, on or before October 8, 2013, and at times prior thereto, instructors did not always wear the Red Man Training Gear face cage and impact resistant eye protection when they trained agents on the proper use of a baton in defensive tactics training. The warning label on the Red Man Training Gear head gear stated: Wear face cage and impact resistant eye protection.  Read all warnings and instructions in product guides.The product guides reiterate in several locations the requirement to wear the face cage and eye protection.

29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness had occurred:  At the establishment the employer did not have an OSHA 300 Log for 2013.  The employer stated that during defensive tactics training in the seventh floor mat room a shoulder injury, a chest injury and an eye injury had occurred in the last six months that resulted in in days away from work, restricted work activity, or medical treatment.
