29 CFR 1915.71(k)(1): Access from below to staging more than 5 feet above a floor, deck or the ground did not consist of well secured stairways, cleated ramps, fixed or portable ladders meeting the requirements of Section 1915.72 or of rigid type non-collapsible trestles with parallel and level rungs.   On or about 26 September 2013, on a floating dry dock at 3820 Lake Palourde Road, Amelia, LA; a welder/fitter was required to access staging by climbing a scaffold without a ladder or other means of access to staging as required by 29 CFR 1915.71(k)(1).  While climbing up and down the scaffold to access staging, the welder/fitter was exposed to a five to ten foot fall.
