29 CFR 1910.1200(e)(2)(ii): The employer's hazard communication program did not include methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the workplace's normal operations and in foreseeable emergencies:  At a production facility in Karnes County, TX, the employer failed to inform employees of other employer's what precautionary measures would need to be taken when attempting to unclog a problematic H2S scrubber. Employees were exposed to scalding hazards from heated water and vapors from calcified H2S and residual H2S.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  On or about September 25, 2013, and at other times prior thereto, at this facility site, the employer failed to have a written certification of a hazard assessment to determine what type of personal protective equipment was required to protect the employees when assisting with a problematic, chemical plugging of an Ultrafab H2S scrubber unit.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:    At a production facility in Karnes County, TX,, Karnes County Texas, where employees, who were performing cleaning operations on a JUltrafab H2S scrubbeer unit, were not protected from a mixture of hot water and spent chemicals propelled out of the Ultrafab vessel directly onto employees due to buillt up water pressure, resulting in a catastrophic incident.
