29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    On or about September 16, 2013, at the above addressed jobsite, employees were heating Ethanol SDA 40-A 190 proof with a kettle and a mixer which were not intrinsically safe for Class I Division 1 flammable vapor atmospheres and the mixture caught on fire.
