29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Back side of roof, corner of Atlanta Road and Fleming Street/An employee working on the back side of the roof was exposed to a fall of approximately 8 feet 2 inches to the ground because no fall protection system was in use.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Back side of the roof, corner of Atlanta road and Fleming Street / Employee(s) were using a step ladder to access the roof that was not 3 feet above the roof edge and/or secured to the structure to prevent movement exposing employees to a 8 feet 2 inches fall hazard.
